I Have a Forged Check, Now What?
In order to file a forgery report, you must fill out a Financial Crimes Self-Report which can be picked up at the Police Department.
Once you complete the report, you should either mail or deliver the report along with any supporting evidence (such as the forged check) to the Police Department.
Guidelines for Completing the Financial Crime Self-Report Form
The form should be completed and turned in within 60 days of the offense.
The account holder must complete a Forgery Affidavit and attach it to the report.
The form must be completed for each check presented.
Do not combine the information and evidence from multiple events into one report.
If you have any surveillance video or photos that might help identify the suspect, please include them with the report. In order for HVPD to investigate a forgery by check, it must have a Thumbprint Signature on it.
